Our congregation recently reviewed our DPA's and got a lot of clarification on issues, as our presiding overseer is on the HLC, and has been for years.
Concerning the selection "c" for item 3, remember that choosing that option insures that your physician will discuss the options with you. Medicine is changing rapidly, and what is used now may have better non-blood options in the near future. Selecting "b" will limit you to exactly what you state. If new options become available later, they will not be available to you if they are not listed in "b". Marking "c" gives you all of the currently available options, they must be discussed with you before use.
